Your First Visit in Practice Centre

On your first visit please arrive 10-15 minutes early for your appointment for registration and to complete a health questionnaire.

When you come for your appointment please remember to bring the following:

 

  • All contact details, medicare card, private health insurance details

  • Referral letter

  • All x-rays and scans (MRI’s, CT, etc.)

To Cancel Your Appointment

We understand that at times appointments need to be cancelled or rescheduled. If you need to cancel or reschedule your appointment, please do so by phoning the practice where one of our administration staff will be more than happy to assist you.

 

Please allow at least 1 days’ notice so that we can offer your spot to the patients on our consultation waiting list.

 

It is preferred that cancellations are not emailed to the practice.

 

This practice reserves the right to change its policy regarding cancellations without notice.
